Good teams rarely develop on their own. Even if they initially work well, it doesn't stay that way. So it is important that team leaders and members take time regularly to review and make adjustments to stay in top form. Particularly when problems have become obvious! In general, to be effective this process needs to be done outside of the normal business flow.
Situations that may require team development can be very different: a new team leader, changing team members, new tasks, changed framework conditions ... or simply put: collaboration and team performance are no longer right.
